Velanix Health Terms of Service
1. About us. Velanix Health Ltd, company number 17314741, 218 Warwick Avenue, Littleover, Derby, DE23 6HP, velanixhealth@gmail.com. These terms govern your purchase of our at-home testing kits and related laboratory analysis.
2. What we provide — and important medical disclaimer. We supply at-home sample collection kits which are analysed by our accredited laboratory partner. Our tests and results are provided for information and wellness screening purposes and are not a substitute for professional medical advice, diagnosis or treatment. Always seek the advice of your GP or a qualified healthcare professional about your health and before making any decision based on your results. Never disregard or delay seeking medical advice because of a Velanix Health result. If you think you have a medical emergency, call 999 or NHS 111. Our results do not create a doctor–patient relationship.
3. Eligibility. You must be 18 or over to purchase. Our kits are intended for use in the UK.
4. Test limitations. No test is 100% accurate. Results can be affected by how the sample is taken, storage and transit. Follow the instructions carefully. A “normal” result does not guarantee the absence of disease, and an “abnormal” result should be confirmed with your doctor.
5. Reporting a problem (MHRA Yellow Card). Our kits are medical devices. If you experience a problem, defect or adverse incident with a kit, please tell us at velanixhealth@gmail.com and report it to the MHRA through the Yellow Card scheme at yellowcard.mhra.gov.uk.
6. Pricing and orders. Prices include VAT where applicable. A contract forms when we confirm dispatch of your order.

8. Liability. Nothing limits our liability for death or personal injury caused by negligence, fraud, or anything that cannot lawfully be excluded. Otherwise our liability is limited to the price you paid.
9. Governing law. These terms are governed by the laws of England and Wales, and disputes are subject to the courts of that jurisdiction.
